Obtaining a student ID

Joint MBA students will need to obtain an OHSU student ID before their first term is completed. The ID card will be electronically keyed with access to the classroom space at OHSU’s Center for Health and Healing outside of normal business hours and on weekends.

There are several important steps that need to be completed before one will be able to get their ID card: Please note: The list below is your guide for gathering what you need to obtain your ID badge and what type of ID badge you are qualified for. You will NOT receive your ID badge if you do not bring the required paperwork.

Please bring all items with you to the Customer Service Center.

  • Background Type: OHSU, Certified
    Instructions: Please submit completed form to the Academic Coordinator prior to the date you wish to get your ID card. Please allow 3 business working days for the Background Check to be processed. (** DO NOT FAX THIS FORM DIRECTLY TO PUBLIC SAFETY. It must be signed by your Academic Coordinator before it is sent to Public Safety).

  • HIPAA
    Instructions: Before obtaining your ID Badge, you will need to complete the online HIPAA training. Please bring this certificate with you.

    Instructions: Before obtaining your ID Badge, you will need to complete the online RAU training. Please bring this certificate with you.

Your Academic Coordinator will submit the ID Card Request form to the ID Office once your background check comes back. You MUST bring your certificates of completion for HIPAA and RAU with you when you go to get your ID Card.

If you are an OHSU employee with a current ID card, you may still receive a student ID card. However please note that you may only have the required access you need for your position on one card. Traditionally students who are also employees have opted to keep their necessary access on their employee ID card and obtained a student card as well.


Library Access

Currently, Joint MBA students have access to OHSU’s library system. Once a student is enrolled at OHSU for classes, they will be assigned a library barcode to use for checking books out of the library. More information about the OHSU Library, including location and hours, can be found here.


Sakai Access

Sakai is the name of the online collaboration and learning environment at OHSU. It provides tools that enable instructors and students to communicate online, get course schedules and materials, and create and submit assignments. All of Sakai’s tools are web-based. All that’s required is a computer with a modern web browser and an internet connection. Communications with Sakai are automatically encrypted for privacy and security.

OHSU’s Center for Health and Healing (CHH)

Joint MBA students will be using CHH for their face to face (residency) sessions two weekends a term. We have access to the classroom space on the third floor of the building to meet our program needs. March Wellness, a membership health center is located on the first and second floors, offers a variety of fitness programs to OHSU employees and the surround community.

 

CHH recently became the first medical and research facility in the nation to receive Leadership in Energy and Environmental Design (LEED) platinum certification, the highest designation awarded by the U.S. Green Building Council, which oversees the LEED rating system. It is one of only 30 buildings across the nation to achieve this distinction (OHSU press release: April 19,2007).

CHH is located at 3303 SW Bond Avenue in Portland, OR.


Financial Aid

Student loans are funds that are borrowed and must be repaid over a period of time, usually after you leave school or drop below half-time enrollment. PSU and OHSU participate in the Federal Direct Lending Program, which means that loan funds are borrowed from the federal government instead of a banking institution.

Each student who is interested in receiving financial aid can now complete a Free Application for Federal Student Aid (FAFSA). Once OHSU has received your completed FAFSA, we will contact you if there are any additional documents needed to complete your file. Therefore, it is very important that you report any address changes to our office. When we ask for additional information, please respond as quickly as possible.

You can access the current FAFSA form at www.fafsa.ed.gov. When completing the application, you MUST put the OHSU school code on your FAFSA application for this program:

Outside Resources - Federal regulations require you to report to us if you will receive any outside resources for Summer 2008 through Spring 2009. Outside resources include but are not limited to Graduate Research Assistantships, Stipends, Employee Tuition Benefit, Traineeships, Private Scholarships, Military awards, National Health Service Corps, WICHE funding, Vocational Rehabilitation, Americorps, etc. Exclude sources of aid that are awarded by both Financial Aid Offices such as Pell Grants, Perkins Loan, Stafford Loans, Health Profession Loans, etc.

Scholarship Websites - We encourage students to apply for outside scholarships as a way to decrease student loan debt. The best advice you can follow when searching the web for scholarships is to allow yourself plenty of time. Many sites will ask you detailed questions in order to match you up with potential scholarships. Other times, it takes a bit of searching and exploration. Don’t give up!

The following are some scholarship websites that may help you with your search.

  • Oregon Student Assistance Commission
    OSAC administers over 370 different scholarships. All are listed on this site and application forms can be downloaded. Remember, the application deadline is a postmark deadline of March 1st.
  • FastWeb!
    FastWeb! is a national scholarship search service. It’s FREE!! Over 600,000 scholarships are in their database. It’s easy to use, and they’ll even email you as new scholarships become available.
  • National Health Service Corp (NHSC)

    The National Health Service Corp has scholarship programs available to students in medicine, nursing, dentistry and physician’s assistant studies. They typically cover tuition, fees, books and supplies, and provide a monthly living allowance. The NHSC’s application deadline is in March.
